What the fare buys is the harder of the two bookings. Entry to the refectory is a timed slot of about fifteen minutes with a hard daily cap, released weeks ahead and gone within hours. Operators hold allocations; you generally cannot.
Everything else in central Milan is walk-up. This is not, and a tour holding a slot is the reliable way in when the official releases have gone.
Full names and dates of birth for every traveller are required at booking, and the guards check them against your ID. Two of these listings say so in capitals, because people get it wrong and are refused.
The route runs through the Galleria, Piazza della Scala and Brera, which is the pleasantest twenty minutes in central Milan and the part a first-time visitor would otherwise cross by metro without seeing.

You get a guide, headsets, a held Last Supper reservation and, on most of these, a skip-the-line ticket for the cathedral. The walk between covers the Galleria, La Scala and, on several, Sforza Castle from outside.
What you do not get is the roof. Not one combined fare here includes terrace access and several exclude it in writing; the terraces are a separate purchase, described on the rooftop page. Two of these fares also stay outside the cathedral entirely despite naming the Duomo in their titles, and their cards say so first. And one of them sells the cathedral ticket and the walking tour as options at checkout rather than including them.
Book as far ahead as you can, because availability rather than price is the binding constraint here for most of the year. Give the exact names and dates of birth on the passports you will carry. Arrive early for the refectory: the group is small and latecomers are not admitted.

If the terraces are what brought you to Milan, none of these will get you there. Take a rooftop fare and add the mural separately. And if you have an hour rather than an afternoon, the one-hour express grade is the honest choice: it is only the mural, and its card says so.
